T HE UTERUS is comprised of a fundus, body, and cervix. Anteflexion is the most common configuration for the uterus where the uterine body and cervix are situated anteriorly in relation to the vagina. Retroflexion and retroversion are common variants where this relationship is reversed. The uterus is enveloped by visceral peritoneum, which extends more inferiorly on the posterior aspect of this organ where it maintains an anatomic space between the rectum and the uterus, the rectouterine pouch. Because of its more dependent location, the pouch of Douglas is often the first spot where fluid accumulates and metastases seed. The anterior peritoneal reflection forms the uterovesical pouch. The fallopian tubes extend laterally from the uterus and consist of a cornual, isthmic, and ampullary portion coursing medial to lateral. The fimbria, the lateralmost extent of the fallopian tubes drape over the ovaries and the entire ovarian-tubal unit is suspended by the broad ligaments.
